Job summary

WestCoast Children’s Clinic in Oakland, CA is seeking an EMPOWR Case Manager to provide trauma-informed community-based support for transition-aged youth (16-24). The role combines advocacy, case management, and collaboration with a multidisciplinary EMPOWR team to coordinate housing, education, employment, and health resources.

The position is full-time, hybrid in-person and remote documentation, with duties including risk assessment, crisis intervention and comprehensive care coordination.

Qualifications

• Bachelor’s degree in Psychology, Criminal Justice, Social Work or related field preferred.
• Direct, personal experience with foster care, social services, juvenile justice, mental health, residential treatment or group home placements is highly desirable.
• At least 2 years experience working with system-involved youth.

Responsibilities

• Provide advocacy, case management, and rehabilitation services to transition-age youth.
• Coordinate with parents, social workers, teachers and other adults in the youth’s life.
• Link to tutoring and vocational programs, mentoring, housing and healthcare resources.
• Conduct risk assessments, crisis intervention, and safety planning.
• Ensure Medi-Cal documentation and billing are accurate and timely.
• Deliver services in the community and in-person for youth and families.
