Job summary

Together California is seeking a compassionate Case Worker to support foster youth and transition-aged youth (TAY), with emphasis on sibling connections, placement stability, permanency, and successful transitions to adulthood.

The Case Worker provides individualized case management, advocacy, and coordination with families, DCFS, courts, schools, and behavioral health providers to ensure holistic support for each youth’s emotional, developmental, educational, and independent living needs.

Qualifications

• Bachelor's or Master's degree in Social Work, Psychology, Human Services, or a related field.
• Experience in child welfare, foster care, or case management is preferred.
• Knowledge of trauma, attachment, child development, and family reunification.

Responsibilities

• Manage individualized case plans prioritizing sibling unity, permanency, safety, and well-being.
• Conduct weekly home visits to monitor youth well-being and progress.
• Advocate for services, educational needs, and behavioral health supports.
• Coordinate with DCFS, courts, schools, therapists, Resource Parents, and others.
• Maintain case notes, assessments, and documentation per state requirements.
• Provide support and resources to Resource and Relief Parents.
• Lead and participate in multidisciplinary team meetings (CFT).
• Support trauma-informed, culturally responsive care for sibling groups.
• Monitor placement needs and advocate for stability and positive outcomes.
