Qualifications

• Experience in human services or social work preferred.
• Ability to assess needs and develop ISP per DMH regs.
• Ability to coordinate services and advocate for clients.

Responsibilities

• Develops comprehensive assessment of TAY needs via interviews and records.
• Develops, implements, and monitors ISP with stakeholders and reviews meetings.
• Provides outreach and recovery‑based education to clients and families.
• Maintains MHIS data accurately and timely per rules.
• Documents records, risk, meetings, and service updates.
