The Commonwealth of Massachusetts, Department of Mental Health, is seeking a Child/Adolescent Human Services Coordinator for the Essex North Site Office in Lawrence. The role focuses on promoting clinically appropriate, least-restrictive services that are strength-based and recovery-oriented, delivered near home.

The position requires advocacy, collaboration with families, schools, and agencies, and effective care coordination to promote client rights and access to entitlements.

Qualifications

• Experience in human services or social work, with knowledge of psychosocial problems and recovery-oriented care.
• Ability to complete client assessments and develop ISP with stakeholders.
• Strong report writing and case management documentation skills.
• Bilingual or multilingual fluency is a plus.

Responsabilités

• Aid consumers in obtaining services to live in least restrictive settings.
• Complete assessments and develop Individual Service Plans (ISP) with family involvement.
• Monitor ISP progress and ensure continuity of care with providers.
• Maintain client records, generate billing information, and communicate with agencies.
• Advocate for clients and provide outreach and supportive counseling.
