Job summary

Samaritan Daytop Village is hiring a Targeted Case Manager to support wellness and recovery goals by delivering targeted interventions and facilitating access to community services.

The role requires a Bachelor's in Human Services (Master's preferred), and experience with clients facing severe mental illness and SUD. Bilingual Spanish is a plus; salary is hourly with a strong non-profit setting and dedicated teams.

Qualifications

• Bachelor's Degree in Human Services is required.
• Master's in Social Work is preferred.
• Bilingual Spanish is preferred.
• Minimum 2+ years of experience counseling individuals with substance use disorders and/or mental illness.

Responsibilities

• Provides case management to Health Home eligible clients and adults with severe mental illness.
• Provides individual counseling and facilitates caseload groups.
• Supports community linkages and referrals to social services.
• Offers transitional care and navigation of health and social services systems.
• Advocates for services and provides family support.
