Job summary

Samaritan Daytop Village, Inc. is seeking a Targeted Case Manager to support wellness and recovery goals by delivering targeted interventions and helping clients access essential services in the community.

You will provide case management for Health Home eligible adults with SMI or SUD, conduct individual counseling, facilitate groups, and advocate for needed services. The role emphasizes timely, high-quality care and coordination with the broader care network.

Qualifications

• Bachelor’s Degree in Human Services required.
• Masters in Social Work preferred.
• Bilingual Spanish preferred.
• Minimum 2+ years of experience counseling clients with SUD and/or mental illness.

Responsibilities

• Provides case management to Health Home eligible adults with SMI or SUD.
• Delivers individual counseling and leads groups.
• Strengthens community service linkages and referrals.
• Facilitates transitional care and navigation of complex services.
• Advocates for client access to needed services.
• Performs crisis intervention when other services are unavailable.
• Monitors treatment plans to address wellness and recovery goals.
• Maintains follow-up with clients, families, and service providers.
