Job summary

Maine Children’s Home is seeking a Targeted Case Manager to provide MaineCare advocacy and service coordination for children, adolescents, and families in home, school, and community settings.

The role requires strong organizational skills, experience with Microsoft Word/Excel and electronic records, and a valid driver’s license. Local travel is required and the position is full‑time and in person.

Qualifications

• Bachelor’s Degree in Social Work, Psychology or related field, with at least 2 years experience
• Strong organizational skills and attention to detail
• Proficiency with Word and Excel; experience with electronic records
• Valid driver’s license and ability to pass background checks

Responsibilities

• Assess and evaluate client needs and complete necessary paperwork
• Meet clients and providers regularly to coordinate services
• Develop individualized service plans and monitor quarterly updates
• Provide information and education about mental health needs and rights
• Advocate for clients and document outcomes in the electronic system
• Meet or exceed billable expectations and participate in supervision and training
