Kids for the Future is hiring a Children’s Targeted Case Manager in Pittsfield, ME. This hybrid, full-time position requires working in clients’ homes and across Somerset, Penobscot, Piscataquis, and surrounding counties.
The role includes conducting assessments, creating care and crisis plans, and coordinating services with schools and providers. The ideal candidate has a 4-year degree in a related field, strong communication, and the ability to travel.
Qualifications

- Strong written and verbal communication skills are essential.
- Must have reliable transportation, valid license, and insurance.
- Ability to travel throughout the community and provide home-based services.
- Ability to work collaboratively while managing an independent caseload.
- Willingness to complete 20 hours of annual case management training.
Responsibilities
- Conduct comprehensive assessments of client and family needs.
- Develop individualized care plans and crisis plans.
- Complete CANS assessments every 90 days.
- Coordinate and monitor community-based services and referrals.
- Collaborate with providers, schools, healthcare professionals, and community agencies.
- Support children and families using trauma-informed, strengths-based practices.
- Monitor client safety, well-being, stability, and progress toward goals.
- Maintain accurate documentation in compliance with laws and policies.
- Manage an independent caseload with scheduling and documentation time.
- Participate in supervision, trainings, and professional development.
- Meet a weekly expectation of 25 billable hours.

As a Children's Targeted Case Manager, you'll work with children and youth from birth through age 21 who are experiencing mental health challenges and/or developmental disabilities. You'll partner with families, schools, healthcare providers, and community agencies to ensure children receive the support and services they need to succeed.

As a Children's Targeted Case Manager, you will:
- Conduct comprehensive assessments of client and family needs.
- Develop individualized care plans and crisis plans.

- Collaborate regularly with providers, schools, healthcare professionals, and community agencies.
- Support and empower children and families using trauma-informed, strengths-based practices.
- Monitor client safety, well-being, stability, and progress toward goals.
- Maintain accurate documentation and client records in compliance with agency policies, MaineCare regulations, licensing requirements, and applicable laws.
- Manage an independent caseload, including scheduling appointments, meetings, and documentation time.
- Participate fully in supervision, trainings, and professional development.
- Meet a weekly expectation of25 billable hours.
- Bachelor's degree in Psychology, Social Work, Mental Health & Human Services, Nursing, Sociology, Education, Behavioral Sciences, or a related field.
- Strong written and verbal communication skills.
- Reliable transportation, valid driver's license, and proof of automobile insurance.
- Ability to travel throughout the community and provide services in clients' homes.
- Ability to work collaboratively while managing an independent caseload.
- Willingness to complete the required20 hours of annual case management training.
- Two or more years of case management experience.
- Knowledge ofSection 13 of the MaineCare Benefits Manual.
- Experience with Microsoft Office, Outlook, Zoom, and other virtual meeting platforms.
- Provide services in clients' homes and throughout the community inSomerset, Penobscot, Piscataquis, and surrounding counties.
- Hybrid work environmentwithWednesdays required in our Pittsfield office.
- Must be comfortable working in homes where pets are present and where cigarette smoking may occur when staff are not present.
