Job summary

You will manage a caseload of up to 25 clients, collaborate with a multidisciplinary team, and document assessments, plans, and progress in accordance with Kentucky Medicaid regulations. The role emphasizes person-centered care and community integration.

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, Human Services or related behavioral health field.
• Eligibility to provide Targeted Case Management services under Kentucky Medicaid regulations.
• Completion of Kentucky Targeted Case Management training within six months of hire.
• Excellent interpersonal, organizational, and communication skills.
• Strong technology skills, including proficiency with electronic health records and virtual communication platforms.
• Ability to work independently while collaborating with a multidisciplinary team.
• Reliable transportation, valid driver’s license, and willingness to travel within the service area.

Responsibilities

• Accept referrals following completion of a diagnostic assessment by a Lumera Healthcare provider.
• Complete comprehensive Targeted Case Management assessments and required reassessments per Kentucky Medicaid regulations.
• Collaborate with clients to develop individualized, strengths-based treatment plans that reflect their goals and identified needs.
• Provide ongoing Targeted Case Management services that promote stability, recovery, and increased independence.
• Monitor client progress and revise treatment plans as needed to address changing circumstances.
• Coordinate care with internal providers and external community partners to ensure continuity of services.
• Connect clients with housing, healthcare, transportation, education, employment, financial assistance, and other resources.
• Advocate for clients while promoting self-advocacy and increased independence.
