Job summary

Isaiah House Treatment Center in Kentucky seeks a compassionate Targeted Case Manager (TCM) to support clients throughout recovery by connecting them with needed services and resources.

You will coordinate the full continuum of case management, develop individualized service plans, monitor progress, and collaborate with treatment teams, community providers, and clients to promote stability and independence.

Qualifications

• Bachelor\'s degree in a related behavioral health, social services, or healthcare field.
• Minimum of 1 year of post-graduate professional experience.
• Strong organizational and time-management skills.
• Excellent verbal and written communication skills.
• Knowledge of Medicaid requirements is a plus.

Responsibilities

• Manage assigned caseloads and develop individualized service plans.
• Coordinate referrals, resources, appointments, and communications among providers.
• Advocate for clients and help them understand services and treatment options.
• Maintain accurate case documentation and reports.
• Monitor progress and adjust services to support positive outcomes.
• Maintain client confidentiality and comply with applicable regulations.
