Job summary

FamilyCare Behavioral Wellness Center in Kentucky offers a Targeted Case Manager (TCM) position that blends office-based case management with fieldwork in community settings. The role supports children, adolescents, and adults by coordinating services, resources, and care plans while navigating Medicaid requirements.

You will work closely with therapists, families, schools, and community partners to implement individualized plans, monitor progress, and advocate for needed services.

Qualifications

• Bachelor's degree in a human services or related field is required.
• Strong written and verbal communication skills are required.
• Proficiency with computers and electronic documentation.
• Valid driver's license and ability to travel throughout the community.

Responsibilities

• Caseload management and client engagement for ~25 clients.
• Complete assessments, care plans, and documentation.
• Coordinate services with therapists, schools, and community partners.
• Maintain timely documentation and meet program requirements.
