Job summary

Catholic Charities is seeking a Targeted Case Manager / Group Facilitator in New Haven to support individuals with behavioral health needs. This hybrid role combines case management with facilitation of Explore and FVEP groups, helping clients access resources, build skills, and progress toward personal goals.

The ideal candidate has a bachelor's degree, a valid driver's license, and at least two years of relevant experience.

Qualifications

  • Bachelor's degree or higher in a human services, behavioral health, social work, psychology, criminal justice, or related field.
  • Must have driver’s license and be able to transport clients to and from appointments as needed.
  • Two or more years of relevant experience in behavioral health, case management, community-based services, group facilitation, or a related setting preferred.
  • Experience facilitating psychoeducational or behavioral health groups.
  • Strong organizational, interpersonal, written, and verbal communication skills.
  • Works collaboratively across programs and disciplines to support coordinated, culturally responsive, and person-centered services.
  • Familiarity with trauma-informed care, motivational interviewing, goal setting, and recovery-oriented practices preferred.
  • Ability to document services accurately and timely in an electronic health record.
  • Proficiency with Microsoft Office applications, including Word, Outlook, and Excel.
  • Written and oral fluency in Spanish strongly preferred.

Responsibilities

  • Assesses client strengths, needs, daily living skills, and barriers to recovery and develops individualized goals.
  • Provides direct, face-to-face skills building in clinic, home, and community.
  • Advocates with and on behalf of clients to reduce barriers to care and increase access to services.
  • Participates in treatment planning and care coordination to ensure services meet client goals.
  • Provides client transportation to support access to care and resources.
  • Facilitates structured Explore and FVEP groups in accordance with curricula.
  • Maintains professional boundaries and completes documentation and reporting.

POSITION SUMMARY

Catholic Charities is seeking a Targeted Case Manager / Group Facilitator to join our New Haven Certified Community Behavioral Health Clinic (CCBHC). This dynamic hybrid role combines targeted case management for individuals with behavioral health and co-occurring needs and assists with facilitation of Explore and Family Violence Education Program (FVEP) groups. The ideal candidate will enjoy working both independently and as part of a collaborative team to help clients build skills, access community resources, overcome barriers, and make meaningful progress toward their goals.

GENERAL DUTIES AND RESPONSIBILITIES
Targeted Case Management
  • Provides targeted case management and psychosocial rehabilitation services to individuals with serious mental illness (SMI) and co-occurring disorders.
  • Assesses client strengths, needs, daily living skills, and barriers to recovery and works with the client and treatment team to develop individualized goals and interventions.
  • Provides direct, face-to-face skills building in the clinic, home, and community, including support with daily living skills, health and wellness, interpersonal relationships, employment, finances, and navigating community resources.
  • Advocates with and on behalf of clients to reduce barriers to care and increase access to needed services and supports.
  • Participates in treatment planning and interdisciplinary care coordination to ensure services remain responsive to each client's goals and needs.
  • Provides client transportation as necessary to support access to care and community resources.
Explore / FVEP Group Facilitation
  • Facilitates structured Explore and Family Violence Education Program (FVEP) groups in accordance with program requirements and established curricula.
  • Creates a respectful, structured group environment that promotes participation, accountability, skill development, and positive behavioral change.
  • Maintains appropriate professional boundaries while engaging participants who may have varying levels of motivation for services.
  • Completes required documentation, attendance records, assessments, and other program-related reporting in accordance with agency and program standards.
  • Communicates and collaborates with program leadership and referral sources regarding participation and completion requirements, as appropriate.
Additional Responsibilities
  • Completes required assessments, outcome measures, and documentation within established timeframes.
  • Participates in supervision, staff meetings, interdisciplinary team meetings, and required trainings.
  • Performs other duties as assigned consistent with the responsibilities of the position.
