Job summary

Easterseals PORT Health in Greenville, NC seeks a compassionate Tailored Care Manager to support individuals by evaluating needs, crafting care plans, and coordinating services addressing physical and mental health and social determinants. The role involves travel throughout Greenville and Winterville, NC, to close gaps in care and connect clients with community resources.

This full-time position offers a Monday–Friday 9 AM–5 PM schedule with rotating on‑call responsibilities, a salary of

Qualifications

• Bachelor’s degree in health, psychology, sociology, social work, nursing or related human services.
• QMHP credentialing eligibility is required.
• Minimum 2 years of experience with mental health and/or substance use populations.
• Proficiency in Microsoft Office and Electronic Health Records (EHR).
• Valid driver’s license, current auto insurance, and a clean driving record.
• Strong communication skills and ability to work independently.

Responsibilities

• Evaluate client needs and develop personalized care plans that address physical and mental health and social determinants of health.
• Coordinate services and resources to close gaps in care for individuals and families.
• Travel throughout Greenville, NC and Winterville to deliver in‑person supports.
• Participate in rotating after‑hours on‑call support.

Your Role in Our Mission

Easterseals PORT Health’s Tailored Care Management (TCM) team is committed to delivering whole-person, integrated care that truly makes a difference. As a Tailored Care Manager, you’ll play a vital role in supporting individuals by evaluating their needs, developing personalized care plans, and coordinating services that address physical and mental health, social determinants of health, and other life challenges. This is a meaningful, hands‑on role with the opportunity to travel throughout Greenville, NC and Winterville, NC helping close gaps in care and improving lives in the community every day.

What We’re Looking For

We’re seeking a compassionate, driven professional with a passion for helping others and connecting clients to vital community resources. This role requires flexibility to provide services in various community settings and participate in rotating after‑hours support.

We also require:

• Bachelor’s degree in a field related to health, psychology, sociology, social work, nursing or another relevant human services area. Master’s level degree preferred.
• Eligible for Qualified Mental Health Professional (QMHP) credentialing
• Minimum 2 years of experience with mental health and/or substance use populations
• Proficiency in Microsoft Office and Electronic Health Records (EHR)
• Valid driver’s license, current auto insurance, and a clean driving record
• Strong communication skills and the ability to work independently
