Job summary

Easterseals PORT Health is hiring a Tailored Care Manager in Goldsboro, NC. You will evaluate needs, develop personalized care plans, and coordinate services that address health, social determinants, and life challenges.

Travel throughout Goldsboro and nearby counties is required, with rotating after-hours coverage. The role offers a Monday–Friday schedule from 8 AM to 5 PM, a competitive salary, and comprehensive benefits for eligible positions.

Qualifications

• Bachelor’s degree in health, psychology, sociology, social work, nursing or related human services field.

Responsibilities

• Evaluate individuals’ needs and develop personalized care plans.
• Coordinate services addressing physical and mental health and social determinants of health.
• Travel throughout Goldsboro, NC and surrounding counties to close gaps in care.
• Provide rotating after-hours support as needed.

Your Role in Our Mission

Easterseals PORT Health's Tailored Care Management (TCM) team is committed to delivering whole-person, integrated care that truly makes a difference. As a Tailored Care Manager, you'll play a vital role in supporting individuals by evaluating their needs, developing personalized care plans, and coordinating services that address physical and mental health, social determinants of health, and other life challenges. This is a meaningful, hands-on role with the opportunity to travel throughout Goldsboro, NC and surrounding counties helping close gaps in care and improving lives in the community every day.

What We're Looking For

We're seeking a compassionate, driven professional with a passion for helping others and connecting clients to vital community resources. This role requires flexibility to provide services in various community settings and participate in rotating after-hours support.

We Also Require

• Bachelor's degree in a field related to health, psychology, sociology, social work, nursing or another relevant human services area. Master's level degree preferred.
• Eligible for Qualified Mental Health Professional (QMHP) credentialing
• Minimum 2 years of experience with mental health and/or substance use populations
• Proficiency in Microsoft Office and Electronic Health Records (EHR)
• Valid driver's license, current auto insurance, and a clean driving record
• Strong communication skills and the ability to work independently
