Job summary

Easterseals PORT Health is seeking a compassionate Tailored Care Manager to join our multidisciplinary Tailored Care Management team in Plymouth, NC. You will evaluate needs, create personalized care plans, and coordinate services to address physical and mental health, social determinants, and other life challenges.

The role is full-time with a Monday–Friday 8 AM to 5 PM schedule and rotating on-call duties, offering benefits including PTO, holidays, comprehensive insurance, retirement plan, EAP

Qualifications

• Bachelor’s degree in a health/human services field; QMHP credentialing.
• Minimum 2 years of experience with mental health and/or substance use populations.
• Proficiency in Microsoft Office and EHR systems.
• Valid driver's license, auto insurance, and clean driving record.
• Strong communication and ability to work independently.

Responsibilities

• Evaluate clients' needs and develop personalized care plans.
• Coordinate services addressing physical/mental health and social determinants of health.
• Travel throughout Plymouth, NC and surrounding counties to close gaps in care.

Your Role in Our Mission

Easterseals PORT Health’s Tailored Care Management (TCM) team is committed to delivering whole-person, integrated care that truly makes a difference. As a Tailored Care Manager, you’ll play a vital role in supporting individuals by evaluating their needs, developing personalized care plans, and coordinating services that address physical and mental health, social determinants of health, and other life challenges. This is a meaningful, hands-on role with the opportunity to travel throughout Plymouth, NC and surrounding counties helping close gaps in care and improving lives in the community every day.

What We’re Looking For

We’re seeking a compassionate, driven professional with a passion for helping others and connecting clients to vital community resources. This role requires flexibility to provide services in various community settings and participate in rotating after-hours support.

We also require:

• Bachelor’s degree in a field related to health, psychology, sociology, social work, nursing or another relevant human services area. Master’s level degree preferred.
• Eligible for Qualified Mental Health Professional (QMHP) credentialing
• Minimum 2 years of experience with mental health and/or substance use populations
• Proficiency in Microsoft Office and Electronic Health Records (EHR)
• Valid driver’s license, current auto insurance, and a clean driving record
• Strong communication skills and the ability to work independently
