Job summary

St. Luke’s in Idaho is seeking a dedicated Case Manager for our Virtual Care Center at Boise St Luke’s Plaza to deliver Hospital at Home services and coordinate acute-level care in patients’ homes.

We offer 8-hour shifts, Monday–Friday 12-8:30 PM, intensive onboarding, full benefits including hospital contributions, and sign-on bonus eligible. Idaho RN license and 2 years RN experience required.

Qualifications

• RN with Idaho license required.
• 2 years of RN experience.
• Education must include an RN degree (Assoc or Bachelor's).

Responsibilities

• Provide case management in the Virtual Care Center in Boise.
• Hospital at Home care model to reduce inpatient stays.
• Participate in intensive onboarding and training.
• Maintain flexible schedule: 8-hour shifts, Mon-Fri 12-8:30 PM.
• Eligible for sign-on bonus and certific pay.

• Hospital at Home is an alternative to traditional inpatient care
• New and innovative care model that will offer acute-level care to patients within their own homes vs in the hospital - providing an enhanced patient experience with reduced costs and improved outcomes
• 8 Hour shift
• Monday-Friday, 12-8:30 PM
• Intensive onboarding and training
• Full Benefits with hospital contributions
• Cert Pay available for qualifying certifications

Qualifications:

• Education: Associates’ of Nursing Degree/Diploma Bachelors’ of Nursing Degree
• Experience: 2 years’ of RN experience
• Licenses/Certifications: Current RN License in Idaho

Preferred Experience in Home Health, community based case management, transition of care, or community complex case experience.
