Job summary

St. Luke’s Health System in Boise invites registered nurses to join our Virtual Care Center. As a Case Manager, you will contribute to Hospital at Home initiatives, delivering acute-level care in patients’ homes with support from a dedicated team.

You will work 8-hour shifts with weekend coverage and enjoy onboarding, full benefits, and potential certification pay. Idaho RN license and 2 years of RN experience are required.

What You Can Expect:

• Position located within our Virtual Care Center at the Boise St Luke's Plaza

• Hospital at Home is an alternative to traditional inpatient care

• New and innovative care model that will offer acute-level care to patients within their own homes vs in the hospital - providing an enhanced patient experience with reduced costs and improved outcomes.

• 8 Hour shift

• Weekend schedule-Saturday and Sunday 8-4:30

• Intensive onboarding and training

• Full Benefits with hospital contributions

• Cert Pay available for qualifying certifications

Qualifications:

• Education: Associates’ of Nursing Degree/Diploma Bachelors’ of Nursing Degree

• Experience: 2 years’ of RN experience

• Licenses/Certifications: Current RN License in Idaho

Preferred Experience in Home Health, community based case management, transition of care, or community complex case experience.
