• Plan individualized care with patient, family, and team involvement and coordinate education and referrals as needed.
• Implement care through delegation and collaboration, managing time, supplies, and resources to meet the plan of care.
• Evaluate patient responses to interventions and adjust the plan as needed, ensuring ongoing reassessment.
• Promote professionalism, adhere to legal/ethical standards, document accurately, and support ongoing learning within licensure scope.
