Qualifications

Responsibilities

• Performs holistic patient assessments using the Christ Hospital model.
• Coordinates care with physicians and team using SBAR handoffs.
• Initiates discharge planning and ensures safety and appropriate referrals.
• Documents care accurately in the EMR and maintains records.
• Leads, mentors, and participates in unit performance initiatives.
