Credentialing Information:

• Credentialing Timeframe: 60-90 days (Typically 30 days if responsive)

• EMR Training: Virtual training required prior to start date, with possible onsite training looped into the first day.

Requirements:

• Board Certification in Vascular Surgery - Required

• Active CO License or IMLC (LOQ in hand at time of submission) - Required

• DEA License - Required

• Availability: Must disclose available shifts from the listed dates and provide ongoing availability.

• Clean Malpractice/Background - Highly Preferred

• EMR Training: Must acknowledge and agree to complete EMR training requirements before start.
