The role of this position involves the operation of a Mobile Health van to assist in patient care. Patients who have mobility or transportation issues, who have not been seen by their PCP for greater than 6 months, who need chronic care management, lab work, medication management, hospital follow-up, or other reasons for continuation of care, may receive the Mobile Health services. The referred patient is visited in their home by a RN Clinical Coordinator for a basic assessment. The RN will facilitate a virtual visit with a clinician. All nursing skills are required as well as the ability to perform lab draws. The candidate must have a clean driving record in order to operate a Carilion owned vehicle. The service area for this position covers the Roanoke, New River Valley regions including the Wytheville and Tazewell areas.

What We Require:Education: Graduate of an accredited School of Nursing required. Bachelor’s degree Preferred.
Experience: Minimum of 3 years RN experience required. At least 2 years experience in relevant specialty or department, as defined by the department, desired. (ex: hospital setting, vascular, rehab, long term care, home health, cardiology, ambulatory, surgery)
Licensure, certification, and/or registration: Current VA RN license required. Must successfully complete orientation and competency requirement.
Must obtain and maintain appropriate certifications as defined by the department.
Valid Virginia, or state specific, drivers license required for employees working in Home Health or departments where the work requires traveling.
Other Minimum Qualifications:
Effective interpersonal, communication, and team-working skills required. Strong critical thinking, conflict resolution, organization and effective judgement abilities required. Must successfully complete orientation and competency validation requirements. Must be able to meet essential job functions. Designated areas may require Knowledge of Medicare/Medicaid and other third party payor regulations
