Job summary

Hackensack Meridian Health is seeking a Patient Navigator, Surgery, to advocate for patients and families through the surgery and cancer continuum. The role emphasizes coordination, education, and timely access to care, with responsibilities spanning scheduling, records preparation, and collaboration with physicians and care teams.

The position requires an NJ RN license and at least 3 years of nursing experience in Surgery/Oncology, plus strong communication and organizational skills.

Qualifications

• RN nursing licensure in NJ or clearly eligible.
• Minimum of 3 years nursing experience in Surgery/Oncology.
• Proficient computer and ECG/EMR skills.
• Strong written and verbal communication abilities.
• Excellent organizational and critical thinking skills.

Responsibilities

• Advocate and educate patients and families through the Surgery/cancer continuum.
• Coordinate appointments and triage patient calls and scheduling.
• Ensure all patient records, orders, films, and labs are ready for visits.
• Provide patient and family education and resources.
• Support care coordination with physicians and care teams.
• Connect patients with resources and social support as needed.
• Contribute to community education and participate in practice hours as required.
