UF Health in Saint Augustine, FL, seeks a Supervisor of Care Coordination to lead and integrate patient care across the care continuum. You will guide the Interdisciplinary Care Coordination Team and manage hand-offs among pharmacists, social workers, physicians, and home health agencies within the acute care setting.
The role requires an active Florida nursing license and BLS. The position emphasizes supervision, delegation, and staff development to ensure high quality care and satisfaction.
The Supervisor of Care Coordination provides effective leadership support by the delegation, coordination and integration of services within their assigned departments to assure the provision of quality care. Serves as a resource for the Care Coordination Team and is responsible for maintaining productive workflows toward the goals of the program. This position reports directly to the Administrator of Medical Surgical and Care Coordination Services and provides supervision and direct support to their teams.
Education Required: Graduate Accredited School of Nursing
Preferences: BSN. In lieu of BSN, commitment to earn BSN within agreed upon time frame may be required.
Experience Requirements: 3 to 5 years Clinical Acute Care
Licensing Requirement: Active FL Nursing License, Basic Life Support (BLS)
BLS provided by the American Heart Association (AHA), required within 30 days of hire.
