UF Health in Florida seeks a Supervisor of Care Coordination to provide leadership in coordinating patient care within the Medical Surgical and Care Coordination Services. This role guides the interdisciplinary team, reconciles plans of care, and ensures efficient hand-offs across care settings.
The position requires graduation from an accredited School of Nursing, Florida RN license with BLS, and 3–5 years of acute care experience. A BSN is preferred or a commitment to obtain one.
The Supervisor of Care Coordinationn provides effective leadership support by the delegation, coordination and integration of services within their assigned departments to assure the provision of quality care. Serves as a resource for the Care Coordination Team and is responsible for maintaining productive workflows toward the goals of the program. This position reports directly to the Administrator of Medical Surgical and Care Coordination Services and provides supervision and direct support to their teams.
Essential Responsibilities:
Education Required: Graduate Accredited School of Nursing
Preferences: BSN. In lieu of BSN, commitment to earn BSN within agreed upon time frame may be required.
Experience Requirements: 3 to 5 years Clinical Acute Care
Licensing Requirement: Active FL Nursing License, Basic Life Support (BLS)
BLS provided by the American Heart Association (AHA), required within 30 days of hire.
