Qualifications

• Bachelor's degree required; Master’s preferred.

• Valid Michigan driver's license; MichiCANS cert within 90 days of hire.
• Excellent communication and report-writing skills.

Responsibilities

• Develop and review the Person-Centered Plan for each participant and coordinate supports.
• Monitor plan implementation and progress; provide referrals as needed.
• Liaise with guardians, staff, and community supports to promote welfare and access services.
