Qualifications

• Proficiency in Microsoft Office required.
• LMSW or LPC licensed.

Responsibilities

• Coordinate and monitor person-centered planning for an assigned caseload.
• Complete monthly supports coordination contacts and clinical notes.
• Review and amend goals per the person-centered plan at identified intervals.
• Maintain clinical records ensuring consents and physicals compliance.
