Job summary

Saginaw County Community Mental Health (SCCMHA) is seeking a Support Coordinator to coordinate services for consumers, develop individualized plans, and monitor outcomes using person-centered planning and recovery-based practices.

Requires a Bachelor’s degree in a mental health field, valid Michigan driver’s license, and licensure options LLBSW/LBSW. Prefer experience with developmental disabilities and collaboration with a range of health and social services.

Qualifications

• Bachelor’s degree in a mental health related field from an accredited school.
• Valid Michigan Driver’s license with a good driving record.
• Experience with individuals with developmental disabilities or mental health populations preferred.
• Licensure options LLBSW/LBSW or eligibility.

Responsibilities

• Develops and monitors service plans with outcomes and supports.
• Coordinates inter-agency resources and community integration.
• Facilitates planning meetings and documents progress.
• Provides crisis intervention and supports to families as needed.
• Communicates with physicians, nurses and therapists.
