Job summary

Center for Community Resources is seeking a full-time Supports Coordinator to serve Pittsburgh-area individuals in Allegheny County. The role will involve face-to-face visits, driving throughout the county, and coordinating community living supports.

The ideal candidate has a Bachelor’s degree with 12 credits in social services or 12 credits plus two years of related experience, plus Act 33/34 and FBI clearances, and a valid driver’s license.

Qualifications

• Education: Bachelor’s degree with 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or related social services.
• OR at least 12 college credits in listed fields AND two years’ professional experience in developmental disabilities, mental health, or related services.
• At least one year’s experience working with individuals with disabilities is preferred.
• ACT 33/34 and FBI Clearances, valid driver’s license and proof of car insurance must be provided prior to hire.
• Must maintain confidentiality and be well organized with strong time management.

Responsibilities

• Develop and maintain the individual support plan using a person-centered approach.
• Inform individuals about service and provider choices and assist in selecting qualified providers.
• Coordinate community living supports including residential, vocational, employment, educational, and social services.
• Provide community activities and support to individuals and families.
• Assess progress toward goals and ensure services meet needs.
• Respond to incidents to protect wellbeing, health, safety, and rights.
