Job summary

Merakey seeks a Supports Coordinator to manage a caseload in Lackawanna, Susquehanna and surrounding counties in Pennsylvania. You will help individuals identify needs, strengths and goals, and develop their service plans with input from assessments and supports.

Responsibilities include collaborating with individuals, families and providers, maintaining regular contact, and adjusting supports as needed. Evening/weekend hours may be required for daytime service delivery.

Qualifications

• Bachelor’s degree with at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice or related social science.
• Two years of college (including an associate degree) with 12 eligible credits AND two years' paraprofessional case management experience.
• 12 eligible credits AND three years' paraprofessional case management experience.
• A combination of 12 college credits in related social science and two years' professional experience in related fields.

Responsibilities

• Manage a caseload in Lackawanna, Susquehanna and surrounding counties in PA.
• Assist individuals in identifying needs, strengths, goals and service preferences.
• Develop and implement individual service plans based on assessments.
• Maintain regular communication with individuals to ensure needed supports are in place.
• Flexible hours, including possible evenings/weekends.

Our Supports Coordinator will be responsible for a caseload in Lackawanna, Susquehanna, and/or surrounding counties in Pennsylvania.The main function of this position is to assist and support the individuals in maximizing their \"everyday life\" by identifying individual's needs, strengths, lifetime goals, and preferences in services/supports.

The principles and practices of Positive Approaches and Self-Determination will be the driving influences in the provision of support services/supports. The ideal candidate will collaborate with individuals, families, and providers to ensure individuals are living their best life.The person in this position will have the responsibility of assisting each individual in the development of their individual service plan, based on the results of assessments designed to develop outcomes appropriate to the individual's \"personal vision\".The person in this position will maintain regular communication and contact with each individual in order to ensure that all aspects of needed/necessary supports are intact.

Flexibility of time and work hours is necessary in this position. There may be a need for evening and/or weekend hours, dependent on the needs of the individuals receiving support.

Earn$17 - $23.50/hour.

This is a Fee for Service position with daytime hours.
