Job summary

Merakey is seeking a full-time Case Manager to join the Dual Diagnosis Treatment Team in Huntingdon, PA. Earn $22.65/hour and provide recovery-focused support across community settings, schools, and residential programs.

The ideal candidate will implement person-centered treatment plans, collaborate with a multidisciplinary team, and facilitate skill transfer to promote independence and recovery for adults, youth, and individuals on the autism spectrum.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Criminal Justice, or similar human services required.
• Experience in the behavioral health field working with individuals with mental health/developmental disabilities required.
• Valid PA driver’s license with a clean driving record and reliable transportation required.

Responsibilities

• Assess, plan, and implement recovery treatment and support plans for adults/youth with mental health diagnoses or autism.
• Provide intensive therapeutic services in multiple settings including community living, schools, and community settings.
• Collaborate with the treatment team to transfer skills identified in the recovery plan using a person-centered approach.

Position Details

The Case Manager is responsible for assessing, planning, and skill transferring recovery treatment and support plans for adults and youth with a mental health diagnosis and/or on the autism spectrum. They provide intensive therapeutic services in all types of settings, including but not limited to community living arrangements, schools, community settings, natural support residents, etc. This candidate, in collaboration with other members of the treatment team, implements and transfers skills identified in the recovery treatment and support plan that utilizes a person-centered approach.
