Job summary

Merakey is seeking a Supports Coordinator to join our team in Pennsylvania. This role focuses on coordinating supports to maximize individuals’ everyday life, developing individual service plans, and collaborating with families and providers across Lackawanna and Susquehanna counties.

Qualifications

• Bachelor’s degree with 12 credits in sociology, social welfare, psychology, gerontology, criminal justice or related social science.
• Two years of college (including an associate degree) with 12 credits and two years’ paraprofessional case management experience.
• 12 credits and three years’ paraprofessional case management experience.
• 12 credits in related social sciences plus two years’ professional experience in developmental disabilities, special education, mental health, counseling or health services.

Responsibilities

• Manage a caseload across Lackawanna, Susquehanna and surrounding counties in Pennsylvania.
• Identify individuals’ needs, strengths, goals and service preferences.
• Develop and implement individualized service plans based on assessments.
• Collaborate with individuals, families and providers to ensure supports promote living their best life.
• Maintain regular communication with individuals to ensure supports are in place; adjust as needed.
• Flexibility for evenings and/or weekend hours as necessary.

• Two years of college (including earning an associate degree) with 12 eligible credits AND two years’ experience in a position performing paraprofessional case management functions. Or
• 12 eligible credits AND three years’ experience in a position performing paraprofessional case management functions. Or
• A combination of 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or other related social science and two years’ professional experience in developmental disabilities, special education, mental health, counseling psychology, school psychology, social work or health and rehabilitative services.

Our Supports Coordinator will be responsible for a caseload in Lackawanna, Susquehanna, and/or surrounding counties in Pennsylvania. The main function of this position is to assist and support the individuals in maximizing their "everyday life” by identifying individuals’ needs, strengths, lifetime goals, and preferences in services/supports.

The principles and practices of Positive Approaches and Self-Determination will be the driving influences in the provision of support services/supports. The ideal candidate will collaborate with individuals, families, and providers to ensure individuals are living their best life. The person in this position will have the responsibility of assisting each individual in the development of their individual service plan, based on the results of assessments designed to develop outcomes appropriate to the individual's "personal vision." The person in this position will maintain regular communication and contact with each individual in order to ensure that all aspects of needed/necessary supports are intact.

Flexibility of time and work hours is necessary in this position. There may be a need for evening and/or weekend hours, dependent on the needs of the individuals receiving support.

Earn$17 - $23.50/hour.

This is a Fee for Service position with daytime hours.
