Merakey in Harrisburg, PA is seeking a Mental Health Worker to join our ACT Team in Dauphin County, providing rehabilitation, support services, crisis intervention, and collaboration with families and community partners.

You will travel across Dauphin County using a PA driver's license to support adults with serious mental illness in home and community settings. We offer comprehensive benefits and opportunities to grow within a multidisciplinary team.

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, Human Services, or another behavioral science required; experience with adults with serious mental illness preferred.
• Bachelor's degree in a non-behavioral field with relevant experience may qualify.
• High school diploma with relevant experience acceptable with demonstrated skills.

Responsibilities

• Assist in providing rehabilitation and support services for individuals with mental health and behavioral health diagnoses.
• Support treatment plans, including substance abuse services, education, and family consultation, under clinical supervision.
• Provide crisis intervention services as needed.
• Collaborate with families to offer education, support, and guidance.
• Collaborate with community resources, including healthcare providers, social services, housing agencies, and other support organizations.
• Ensure safety and well-being of consumers in residential programs by following safety protocols.
• Participate in direct care responsibilities, including daily living support and therapeutic activities.
• Follow and contribute to the treatment planning process with the clinical team.

Position Details:

• Assist in providing rehabilitation and support services for individuals with mental health and behavioral health diagnoses.
• Support treatment plans, including substance abuse services, education, and family consultation, under clinical supervision.
• Provide crisis intervention services as needed.
• Collaborate with families to offer education, support, and guidance.
• Collaborate with various community resources, including healthcare providers, social services, housing agencies, and other support organizations, to ensure comprehensive care for clients.
• Ensure the safety and well-being of consumers in residential programs by adhering to safety protocols.

Assertive Community Treatment (ACT) Teams are composed of professionals including psychiatrists, nurses, mental health professionals, substance abuse professionals, employment specialists, housing specialists, and a peer specialist, all of whom collaborate to provide complete services including therapy, medication management, and resource linkage to adult individuals with serious mental illness (SMI). Services are provided in the individual's home or the community and include at least three home visits a week. Will berequiredtoutilizepersonal vehicles for client transportation and community-based work.

• Experience in human services, healthcare, education, or other helping professions is a plus.
• Strong interpersonal, communication, and relationship-building skills.
• Ability to provide rehabilitation, recovery-oriented support, and community-based services as part of a multidisciplinary team.
• Valid PA Driver's License.Travel is necessary to offer support across Dauphin County.
