Job summary

Merakey in Monaca, PA is seeking a Blended Case Manager to join our team. The role involves serving as the primary contact for up to 30 individuals, coordinating personalized service plans, and working with county reentry grants to help individuals transition back into the community.

The ideal candidate has a high school diploma with social science credits and 2+ years in human services, including direct care, plus a valid driver's license.

Qualifications

• High school diploma plus at least 12 semester credits in sociology, social welfare, psychology, gerontology, or other social science.
• 2+ years’ experience in public or private human service, with 1 year direct care.
• Valid Driver's license required.
• All clearances required by OMHSAS regulations.

Responsibilities

• Serve as the primary contact for up to 30 individuals in the community.
• Develop, coordinate, and implement personalized service plans.
• Provide case management, service planning, and coordination with other agency providers.
• Offer supportive counseling to help individuals achieve their goals.
• Work with county reentry grants to support individuals transitioning from jail back into the community.
• Participate in on-call responsibilities as needed.
