Job summary

Merakey in Penn Hills, PA is seeking a full-time Case Manager to join the Dual Diagnosis Treatment Team. You will assess, plan, and implement recovery-focused treatment and support plans for adults and youth with mental health diagnoses or autism spectrum conditions.

The ideal candidate holds a Bachelor’s degree in social work, psychology, or a related field, has a PA driver’s license, and experience in behavioral health.

Qualifications

• Bachelor’s degree in social work, psychology, or related human service field.
• Valid PA driver’s license with clean driving record.
• Experience in behavioral health with mental health/developmental disabilities is required.

Responsibilities

The Case Manager is responsible for assessing, planning, and skill transferring recovery treatment and support plans for adults and youth with a mental health diagnosis and/or on the autism spectrum. They provide intensive therapeutic services in all types of settings, including but not limited to community living arrangements, schools, community settings, natural support residents, etc. This candidate, in collaboration with other members of the treatment team, implements and transfers skills identified in the recovery treatment and support plan that utilizes a person-centered approach.
