Job summary

You will assist in choosing and coordinating services, monitor delivery, and promote individuality, choice and control. The role requires experience with intellectual/developmental disabilities, proficiency with computers and HCSIS, and a commitment to customer service as you work with consumers and families to identify outcomes

Qualifications

• 2 years' Human Services work experience (required)
• Degree: associate or bachelor’s in applicable field with 12 credits in social science coursework
• Proficient with computers and HCSIS
• Valid driver’s license and reliable car

Responsibilities

• Complete assessments to identify needs and desires.
• Develop Individual Support Plans and personal budgets with a person-centered approach.
• Identify outcomes and ensure ISP addresses them.
• Coordinate services and complete referrals; resolve delivery barriers.
• Monitor services to ensure delivery and safety; track outcomes.
• Assist in maintaining the person’s support network (family, friends).
• Arrange and manage generic resources and community supports.
• Maintain compliance with organizational policies and best practices.
• Attend trainings and educate families on rights and due process.
