Job summary

Service Access & Management, Inc. (SAM) seeks a Service Coordinator for its Early Intervention service line in Berks County, PA. You will provide case management for children 3 and under with developmental delays, coordinating services with families and professionals to promote positive outcomes.

Qualifications

  • Bachelor's degree with 12 EI/early education credits and 1 year of related experience.
  • Associate's degree (60 credits) with 3 years of related experience.
  • Ability to manage a dynamic caseload.
  • Strong interpersonal, organizational, and communication skills.
  • Fluent in Spanish, written and verbal.
  • Valid driver's license and reliable transportation.

Responsibilities

  • Request/complete service authorizations.
  • Comply with all OCDEL standards.
  • Conduct Family Service Plan process while ensuring quality and compliance.
  • Participate in Multi-Disciplinary Team to support positive outcomes.
  • Complete referrals to agencies so that children have access to needed support and services.
  • Facilitate Transition Plan to support continuity of care.
  • Complete/update service plan as needed.
  • Maintain positive relationships.
  • Complete written documentation.

About The Service Coordinator Spanish Speaking- Case Manager Role

As a Service Coordinator within our Early Intervention (EI) service line, you'll play a vital role in supporting individuals with EI, providing case management services to children 3 and under diagnosed with a developmental delay. Staff work closely with parents, caregivers, and professionals to support children in reaching their fullest potential. You’ll build long-term relationships through ongoing case management, advocacy, and coordination of meaningful services. This position is ideal for someone who is compassionate, detail-oriented, and thrives in community-based environments where collaboration and relationships matter.

Starting Rate: $24.131/hour, per current union agreement

Service Coordinator - Case Manager Core Responsibilities
  • Request/complete service authorizations.

What You Bring
  • A bachelor's degree from an accredited college or university which includes 12 college credits in early intervention, early childhood special education, early childhood education, child development, special education, family counseling, family studies, social welfare, psychology or other comparable social sciences, and 1 year of full-time or full-time-equivalent experience working with or providing counseling to children, families or individuals with disabilities.
  • An associate's degree, or 60 credit hours, from an accredited college or university in early intervention, early childhood special education, early childhood education, child development, special education, family counseling, family studies, social welfare, psychology, or other comparable social sciences, and 3 years of full-time or full-time-equivalent experience working with or providing counseling to children, families or individuals with disabilities.
  • Ability to work independently and manage a dynamic caseload
  • Strong interpersonal, organizational, and communication skills
  • Fluent in Spanish, written and verbal
  • A valid driver’s license and reliable transportation

Location & Schedule

This position is based in Berks County, PA. Work is primarily community-based with remote flexibility. Office hours are Monday through Friday, 8:00 AM to 5:00 PM.
