Qualifications

• Two years of experience as County Social Services Aide 3 and two years of college credit in sociology, social welfare, psychology, gerontology, criminal justice or related social sciences; or
• Bachelor's degree with 12 credits in sociology, social welfare, psychology, gerontology, criminal justice or related social sciences; or
• Equivalent combination of experience and training including 12 credits in social sciences and one year of paraprofessional case management experience; or
• Educational requirements may be met within two months of graduation.

Responsibilities

• Plan Development: Create treatment plans, life management plans, and Individual Support Plans based on consumer needs.
• Service Coordination: Locate providers and enroll individuals in service programs.
• Record Maintenance: Keep organized files and documentation for continuity of care.
• Consumer Advocacy: Act as a voice for the consumer to ensure appropriate treatment.
• Field Responsibilities: Travel to homes, agencies, and service sites within and outside the county.
• Deadline Management: Complete daily paperwork accurately while meeting changing deadlines.

Education

Bachelor's degree with 12 credits in sociology, social welfare, psychology, gerontology, criminal justice, or related social sciences

2 years experience as County Social Services Aide 3 or similar paraprofessional case management

Equivalent combination of experience and training with 12 college credits in related social sciences

Residency/education proximity to graduation within two months
