Job summary

CCRI in Moorhead, MN seeks a Full-Time Supportive Housing Specialist to provide intensive short- and long-term case management for individuals and families in supportive housing. You will assess needs, develop individualized housing plans, and coordinate access to mental health, medical, social, educational, and vocational services.

You will work with community agencies and landlords to promote housing retention and well-being, ensuring program compliance and documentation.

Qualifications

• Associate degree in social work or related field and at least 1 year of related experience.
• Must have a valid driver's license, current car insurance, pass a background check, and have internet-enabled device.
• Proficiency in speaking and writing in English.

Responsibilities

Full-Time Supportive Housing Specialist

The Supportive Housing Specialist provides intensive short- and long-term case management to individuals and families who reside in supportive housing. This role assesses client needs, develops individualized housing plans, and coordinates access to mental health, medical, social, educational, vocational, and other essential services. The position works closely with community agencies, property managers, and support networks to promote housing retention, independence, and overall well-being, while ensuring compliance with program requirements and maintaining accurate documentation.

Qualifications: Associate degree in social work or related field and a minimum of one (1) year of related experience working with persons with disabilities, persons aged 65 or older, low-income households, homeless populations, substance use and/ or behavioral health programs; OR an equivalent combination of education andexperience.

Must have a valid driver's license, current car insurance, meet insurability standards, pass a background check, and have an internet-enabled device. Proficiency in speaking and writing in the English language is required.

40 hours per week flexible Monday-Friday 8 A.M.-5 P.M.
