Job summary

Goodwill of the Heartland is hiring a Supported Community Living Associate who builds relationships with program participants and connects them with community resources. You will help identify treatment and service needs, monitor progress, assist with ISP development, and write progress reports while helping manage expenses and billable hours.

The role requires a high school diploma or GED, at least six months experience in training or rehabilitation, and a reliable vehicle with a valid Iowa

Qualifications

• High school diploma or GED required.
• Six months experience in training/rehabilitation required; post-secondary education may be substituted for experience.
• Reliable vehicle with valid Iowa driver's license, proof of auto-insurance, and successful completion of background checks.

Responsibilities

• Develop and maintain effective relationships with clients.
• Identify and maintain information about community resources that may benefit program participants.
• Identify treatment and service needs of each individual served.
• Monitor and evaluate progress of the treatment plan and assist as appropriate.
• Monitor and control expenses and ensure efficiency in rehabilitation services, including billable hours and client funding.
• Write quality case notes and assist with ISP development and progress reports.
• Provide transportation to participants as needed.
