Job summary

Toolworks is seeking a Program Case Manager to oversee Supported Living services for adults with disabilities in the San Francisco Bay Area, including San Francisco, San Mateo County, and East Bay. You will supervise staff, coordinate services for a caseload of six to eight clients, and ensure quality and regulatory compliance.

Candidates with an MSW and active California ASW registration are encouraged to apply.

Qualifications

• Bachelor's degree in Special Education, Rehabilitation Counseling, Social Work, Human Services or related field.
• Master of Social Work (MSW) and CA Associate Clinical Social Worker registration are desirable.
• Three years’ experience with adults with disabilities; supervisory experience valued.

Responsibilities

• Emergency response management for clients and staffing, ensuring safety and continuity of care.
• Maintain regular, meaningful contact with clients and their support networks to foster independence.
• Recruit, train, and mentor staff to deliver exceptional support.
• Oversee scheduling, training compliance, and safety procedures for smooth operations.
• Represent Toolworks positively to the community and uphold company values.
