Job summary

Toolworks is seeking an experienced Program Case Manager to oversee Supported Living services for adults with disabilities in San Francisco, San Mateo County, and East Bay. You will supervise Community Living Instructors and Leads, coordinate services for six to eight clients, and monitor quality and regulatory compliance to enhance client independence.

Candidates with an MSW and active California ASW registration are encouraged to apply.

Qualifications

• Bachelor's degree in a relevant field preferred; MSW and active CA ASW registration are highly desirable.
• 3+ years in a role supporting adults with disabilities, with preference for case management experience.
• Supervisory experience valued; strong report writing and time management skills required.

Responsibilities

• Emergency Response: Manage emergencies and ensure safety and continuity of care.
• Client Engagement: Maintain regular contact with clients and support networks to promote independence.
• Staff Leadership: Recruit, train, and mentor team to provide exceptional support.
• Administrative Excellence: Oversee scheduling, training compliance, and safety procedures.
