Job summary

Goodwill of the Heartland is seeking a Supported Community Living Specialist to build strong relationships with participants and referral agencies while coordinating community resources. The role develops treatment plans, monitors progress, and assists with Medicaid documentation, ensuring efficient use of rehabilitation funds.

A valid Iowa driver’s license and background checks are required. The schedule is weekdays, travel between participant locations is required, and the starting wage is

Qualifications

• Bachelor's degree in social work, social science, rehabilitation or related counseling field.
• At least 1 year of related experience required; Master’s degree may substitute for the experience.
• Reliable vehicle and Iowa driver's license, proof of auto-insurance.
• Successful completion of required background checks (MVR, abuse & criminal).

Responsibilities

• Develop and maintain relationships with participants, referral agencies, and funders.
• Identify and utilize community resources benefiting participants.
• Develop and monitor treatment/service plans and evaluate progress.
• Assist and facilitate services as appropriate.
• Manage expenses to ensure efficiency in rehabilitation services and submit Medicaid documentation.

Education

Bachelor's degree in social work, social science, rehabilitation or other counseling related field

Master’s degree may substitute for the year of experience
