Job summary

Goodwill of the Heartland in Cedar Rapids, IA is seeking a Supported Community Living Specialist to help individuals gain independence by coordinating services and building community ties. The role develops and monitors treatment plans, identifies resources, and ensures Medicaid documentation and budgeted expenditures are managed.

Travel between participant locations is required; Monday–Friday, 8:00am–5:00pm.

Qualifications

• Bachelor’s degree in social work, social science, rehabilitation or other counseling related field.
• One year of related experience required. A Master’s degree may be substituted for the year of experience.

Responsibilities

• Develop and maintain relationships with program participants, referral agencies, and funders.
• Identify and utilize community resources to benefit participants.
• Develop and monitor treatment and service plans and evaluate progress.
