Goodwill of the Heartland in Cedar Rapids, IA is seeking a Supported Community Living Specialist to help individuals gain independence by coordinating services and building community ties. The role develops and monitors treatment plans, identifies resources, and ensures Medicaid documentation and budgeted expenditures are managed.

Travel between participant locations is required; Monday–Friday, 8:00am–5:00pm.

Qualifications

• Bachelor’s degree in social work, social science, rehabilitation or other counseling related field.
• One year of related experience required. A Master’s degree may be substituted for the year of experience.

A Bachelor's degree in social work, social science, rehabilitation, or other counseling related field and one year of relevant experience is required for this role. Our Supported Community Living Specialist is responsible for developing and maintaining relationships with program participants, referral agencies, and funders, as well as finding and utilizing community resources that may benefit our program participants. This role helps identify and develop treatment and service plans that meet the needs of each participant, as well as monitoring and evaluating the progress of the treatment plan while assisting and facilitating as appropriate.
