Job summary

Goodwill of the Heartland is seeking a Supported Community Living Specialist to build strong relationships with participants and referral agencies while coordinating community resources. The role develops treatment plans, monitors progress, and assists with Medicaid documentation, ensuring efficient use of rehabilitation funds.

Qualifications

• Bachelor's degree in social work, social science, rehabilitation or related counseling field.
• At least 1 year of related experience required; Master’s degree may substitute for the experience.
• Reliable vehicle and Iowa driver's license, proof of auto-insurance.
• Successful completion of required background checks (MVR, abuse & criminal).

Responsibilities

• Develop and maintain relationships with participants, referral agencies, and funders.
• Identify and utilize community resources benefiting participants.
• Develop and monitor treatment/service plans and evaluate progress.
• Assist and facilitate services as appropriate.
• Manage expenses to ensure efficiency in rehabilitation services and submit Medicaid documentation.

A Bachelor's degree in social work, social science, rehabilitation, or other counseling related field and one year of relevant experience is required for this role.Our Supported Community Living Specialist is responsible for developing and maintaining relationships with program participants, referral agencies, and funders, as well as finding and utilizing community resources that may benefit our program participants. This role helps identify and develop treatment and service plans that meet the needs of each participant, as well as monitoring and evaluating the progress of the treatment plan while assisting and facilitating as appropriate.
