Responsibilities

• Lead and supervise a team of 5+ DSPs, ensuring DDS standards compliance.
• Oversee individualized programs, monitor progress, and conduct monthly reviews.
• Act as a liaison between individuals, families, case managers, and providers.
• Conduct on-site monitoring visits and maintain accurate documentation.
• Serve as Health Care Coordinator or back-up for medical needs per DDS guidelines.
• Ensure ongoing eligibility for benefits, report wages, and monitor bill payments.
• Provide hands-on support including transportation to work, medical appointments, and community activities.
• Promote community engagement and vocational opportunities for competitive employment.
• Provide on-call emergency support for individuals and staff.
