Job summary

Central State Community Services of Oklahoma is seeking a dedicated Support Coordinator to lead a team of Direct Support Professionals providing essential care to individuals with developmental disabilities.

This role requires 24-hour on-call availability, collaboration with families, case managers, and healthcare providers, and strict adherence to DDS standards to maintain high-quality services.

You will ensure timely responses to urgent needs, monitor progress, and promote community engagement.

Qualifications

• Experience in human services, social work, or related field required.
• Strong leadership and organizational skills.

Responsibilities

• Lead and supervise a team of 5+ Direct Support Professionals (DSPs) to ensure DDS standards.
• Oversee individualized programs, monitor progress, and complete monthly reviews.
• Act as liaison between individuals, families, case managers, and healthcare providers.
• Conduct on-site monitoring visits and maintain accurate documentation.
• Serve as Health Care Coordinator (or back-up) per DDS guidelines.
• Manage benefits, wages, and monitor bill payments.
• Provide hands-on support including transportation to appointments and activities.
• Promote community engagement and help individuals find volunteer opportunities.
• Stay on-call to respond to urgent needs and ensure timely response.
