Job Summary
As a Support Coordinator II, you will provide case management support for an assigned caseload. You will help people with disabilities perform their jobs successfully within a production environment or, as needed, assist people in an enrichment class setting. You will help people grow their skills and gain independence to the greatest extent possible.

Responsibilities
  • Provide case management so that services are initiated, provided, evaluated and communicated effectively both internally and externally
  • Enter attendance and work records into our computer system as needed
  • Provide training and instruction to individuals within a production environment or an enrichment setting
  • Monitor work and class activities to ensure they are conducted safely and that independence, integration and inclusion are occurring
  • Maintain a calm, neutral tone and emotional level when assisting people who are expressing heightened communication styles
  • Implement lessons and activities that provide people opportunities to learn and practice skills

Designated Coordinator Qualifications
  • A designated coordinator may have a baccalaureate degree in a field related to human services, education, or health and one year of full-time work experience providing direct care services to persons with disabilities or related populations
  • A designated coordinator may have an associate degree in a related field and two years of full-time work experience providing direct care services
  • A designated coordinator may have a diploma in a related field and three years of full-time work experience
  • A minimum of 50 hours of education and training related to human services, education, or health and four years full-time work experience providing direct care services to persons with disabilities or related populations, under supervision of staff meeting designated qualifications
