Job summary

BlueCross BlueShield of Tennessee, Inc is seeking a dedicated Support Coordinator for ECF Choices in Montgomery County. You will empower members to achieve health and independence by coordinating services, assessments, and person-centered plans.

Travel to members' homes is required, mileage reimbursed. The role features an 8:00am–5:00pm schedule, and you will collaborate with a team to meet physical, behavioral health, and long-term service needs.

Qualifications

• RN with Certification from DD Nurses Association (CDDN) or DD Nurse (DDC) as applicable.
• BS in social work, nursing, education, or health care field.
• Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) preferred.

Responsibilities

• Partner with members and families to identify needed supports and direct services.
• Collaborate with clinical and social support colleagues to meet health and long-term needs.
• Conduct face-to-face visits and assess needs across physical, behavioral, functional, psycho-social, financial and living aspects.
• Ensure appropriate home and community based services are authorized and provided.
