Job summary

Gift of Life Home Health is seeking a Coordinated Family Support (CFS) – Support Coordinator in California. You will provide individualized coordination, advocacy, and resource linkage for adults with developmental disabilities and their families.

The role involves collaborating with Regional Centers, community agencies, and interdisciplinary teams to promote independence, access to benefits, and improved quality of life.

Qualifications

• Bachelor’s degree preferred in social work, psychology, human services, nursing, or related field.
• Experience working with individuals with developmental disabilities and families is preferred.
• Knowledge of Regional Center services, IHSS, SSI/SSA benefits, Medi-Cal, CalFresh, and community resources is preferred.
• Strong organizational, communication, and documentation skills.
• Ability to work independently while maintaining communication with supervisors and team.
• Ability to manage multiple cases and meet deadlines.

Responsibilities

• Participate in intake assessments and assist in identifying client and family needs.
• Follow individualized monthly plans with measurable goals and outcomes.
• Maintain client encounters per week as assigned.
• Coordinate and monitor services to ensure follow-through and continuity of care.
• Assist families with accessing IHSS, SSI/SSA benefits, Medi-Cal, CalFresh, behavioral health, and other resources.
• Provide referrals to legal and financial planning resources when appropriate.
• Provide advocacy and support during appointments and meetings.
• Maintain professional communication with Regional Center staff and community partners.
• Complete service notes reflecting progress, barriers, and next steps.
• Participate in weekly staff meetings, trainings, and quality assurance activities.
